BACKGROUND: Preperitoneal placement of mesh during herniorraphy has been shown to complicate future extirpative prostate surgery. We investigated the value of a prostate cancer screening program in patients considering laparoscopic inguinal herniorrhaphy to identify men at risk of prostate cancer. STUDY DESIGN: A prospective cohort study was conducted in men 30 years of age or older presenting with inguinal hernia. All patients were counseled on prostate cancer risk and the potential for herniorrhaphy with mesh placement to complicate future pelvic surgery. Serum prostate specific antigen (PSA) values were obtained, and patients determined to be at increased relative risk of prostate cancer were referred for urologic evaluation. Transrectal ultrasonography-guided biopsy was performed if clinically indicated, and operative data for patients undergoing prostate cancer treatment were retrospectively reviewed. RESULTS: There were 1,324 patients who presented for management of inguinal hernia; 814 of these (median age 60 years) consented to screening. Overall, 259 (32%) had an increased relative risk of prostate cancer based on PSA 1.0 to 2.49 ng/mL, and 152 (19%) had PSA > or = 2.5 ng/mL. Transrectal ultrasonography-guided biopsy was performed in 86 patients. Prostate cancer was identified on initial or repeat biopsy in 31 patients (3.8%), including 1 patient (0.7%) younger than 50 years of age. CONCLUSIONS: We found the incidence of concurrent prostate cancer with hernia to be low, but 51% of men had PSA values that suggested an increased relative risk of future development of prostate cancer. Men at increased risk of prostate cancer should be made aware of the impact that mesh might have on subsequent treatment options before mesh placement.

Das Netz ist also bei möglichen Folgeeingriffen ein Problem. Das Prostatakarzinom ist eine der häufigsten Krebsformen beim Mann. Sollte man sich dann mit 22 schon unnötig Schwierigkeiten bei der weiteren Versorgung machen, wenn es nicht notwendig ist?

BACKGROUND: Femoral hernias are uncommon, and there are relatively few clinical studies of longterm outcomes after repair. Although the McVay repair has been classically described, the infrainguinal plug technique has gained popularity in recent years. Evidence supporting these repairs is sparse. STUDY DESIGN: A prospective study of elective femoral hernia repairs was done at the Shouldice Hospital from June 1999 until June 2003. The tissue-based complete groin repair (CGR) and a preperitoneal mesh repair were performed for specific indications. Patients were followed annually for 5 years to examine for recurrences and complications. RESULTS: Two hundred fifty-six patients were enrolled, with 225 completing 5 years of followup. Median age was 55 years, and hernias on the right side were more common (63.1%). Concurrent inguinal hernias were found in 115 patients (51%), and 41 (18.2%) had a previous inguinal hernia repair. A complete groin repair was performed in 120 patients and a preperitoneal mesh repair in 78. The remaining had an infrainguinal mesh repair. The overall recurrence rate was 3.1%, with a median time to recurrence of 12 months. There was no significant difference between mesh and suture repairs. Chronic postoperative pain was experienced by 20 patients (8.9%). CONCLUSIONS: Femoral hernias can be repaired electively with a tissue-based or a preperitoneal mesh technique, with durable longterm results. Mesh repair is indicated for recurrent femoral hernias, inguinofemoral hernias, prevascular hernias, association with concurrent direct hernias, and, if tension is anticipated, with complete groin repair. Infrainguinal mesh repair is used only when there has been a successful previous inguinal hernia repair.

Kein Unterschied im Auftreten von Rezidiven zwischen der Technik mit und ohne Netz.
Ich habe jetzt noch zahlreiche andere Studien herausgesucht. Der Grundtenor ist, dass der Unterschied zwischen Leistenbruch-OP mit oder ohne Netz marginal ist. Wenn also kein eindeutiger Vorteil zu erkennen ist, dann würde ich zum risikoärmeren Verfahren neigen.

BACKGROUND: Inguinal hernia repair is the most frequent operation in general surgery. There are several techniques: the Shouldice technique is sometimes considered the best method but different techniques are used as the "gold standard" for open hernia repair. Outcome measures, such as recurrence rates, complications and length of post operative stay, vary considerably among the various techniques. OBJECTIVES: To evaluate the efficacy and safety of the Shouldice technique compared to other non-laparoscopic techniques for hernia repair. SEARCH STRATEGY: We searched MEDLINE, EMBASE, and The Cochrane Central Register of Controlled Trials (CENTRAL), April 2008, for relevant randomised controlled trials. SELECTION CRITERIA: Any randomised or quasi-randomised controlled trials (RCT) on the treatment of primary inguinal hernia in adults were considered for inclusion. DATA COLLECTION AND ANALYSIS: All abstracts identified by the search strategies were assessed by two independent researchers to exclude studies that did not meet the inclusion criteria. The full publications of all possibly relevant abstracts were obtained and formally assessed. Missing or updated informations was sought by contacting the authors. MAIN RESULTS: Sixteen trials contributed to this review. A total of 2566 hernias were analysed in the Shouldice group with 1121 mesh and 1608 non-mesh techniques. The recurrence rate with Shouldice techniques was higher than mesh techniques (OR 3.80, 95% CI 1.99 to 7.26) but lower than non-mesh techniques (OR 0.62, 95% CI 0.45 to 0.85). There were no significant differences in chronic pain, complications and post-operative stay. Female were nearly 3% of included patients. AUTHORS' CONCLUSIONS: Shouldice herniorrhaphy is the best non-mesh technique in terms of recurrence, though it is more time consuming and needs a slightly longer post-operative hospital stay. The use of mesh is associated with a lower rate of recurrence. The quality of included studies, assessed with jaded scale, were low. Patients have similar characteristic in the treatment and control group but seems more healthy than in general population, this features may affect the dimension of effect in particularly recurrence rate could be higher in general population. Lost to follow-up were similar in the treatment and control group but the reasons were often not reported. The length of follow-up vary broadly among the studies from 1 year to 13.7 year.
