Pietinen et al.: Diet and risk of colorectal cancer in a cohort of Finnish men. Cancer Causes Control. 1999 Oct;10(5):387-96:
The results of this study are quite surprising. Intakes of fat or fiber were not related to risk of colorectal cancer, and neither was consumption of meat, vegetables, or whole grain cereals.
Willett: Dietary fat intake and cancer risk: a controversial and instructive story. Semin Cancer Biol. 1998 Aug;8(4):245-53:
For colon and prostate cancer, the data from prospective studies are more limited, but also suggest that fat per se is not a risk factor.
Chatenoud et al.: Refined-cereal intake and risk of selected cancers in Italy. Am J Clin Nutr. 1999 Dec;70(6):1107-10:

This systematic analysis of an integrated series of case-control studies showed a consistent pattern of direct association between frequent intake of refined cereals (mainly bread and pasta, but also rice) and the risk of several common neoplasms, including those of the major digestive tract sites, the larynx, and the thyroid, in this Italian sample. These findings agree with those of several other studies that showed that refined cereals have an unfavorable effect on carcinogenesis of the large bowel, the stomach, the upper digestive tract, and selected other sites; these studies include 2 large case-control studies of breast cancer (20) and colorectal cancer (22, 23) conducted at several Italian study sites and based on a more detailed food-frequency questionnaire.

Similar associations were observed with sugar (29–31), which, from a nutritional viewpoint, confirms the metabolic similarities between sugars and refined cereals. It is known that refined cereals and sugars have a higher rate of digestion than do whole-grain cereals and other components of diet, causing glycemic overload and compensatory increases in plasma insulin concentration and insulin-like growth factor I, an important mitogenic stimulant of tumor cell growth in vitro (32).

Colorectal cancer and the consumption of foods: a case-control study in Belgium.

Tuyns AJ, Kaaks R, Haelterman M.

International Agency for Research on Cancer, Lyon, France.

A case-control study on 453 cases with colon cancer, 365 with rectal cancer, and 2,851 population controls was carried out in two Belgian provinces known to differ in certain dietary habits, particularly with regard to the use of butter. All raw vegetables had a clear protective effect for both colon and rectal cancer; bread was also protective for colon cancer. Starchy foods and foods rich in oligosaccharides (sugar) caused an increased risk for both colon and rectal cancer. No other foods were found to have a systematic effect in both sexes and in both provinces, either in one direction or in the other, except for maize, soybean, and sunflower oils, which were clearly protective in all cases. Among the foods contributing to the intake of fats, there was no effect either for butter, margarine, or fatty meats; the only clear-cut protective effect was that of the oils having a high polyunsaturated-to-saturated ratio. These findings are consistent with our previous findings on the role nutrients play in the relationship with colon and rectal cancers.

Biesalski: Meat and cancer: meat as a component of a healthy diet. European Journal of Clinical Nutrition (2002) 56, Suppl 1, S2–S11:

Several epidemiological early case – control and cohort studies suggested a positive correlation between fat intake and incidences of breast, colon and prostate cancer (Schottenfeld & Fraumeni,1996; Potter et al, 1993). However more recent cohort, large case – control and pooled analysis of 13 case – control studies failed to detect an association between fat intake and colon cancer (Howe et al 1997; Giovannucci et al, 1994).
The relationship between dietary fat intake and breast cancer has been examined in a couple of rospective studies. In a pooled analysis no overall association was seen for total fat intake over the range of 15 to >45% of energy from fat (Hunter et al, 1996). In contrast, among the small number of women consuming less than 15% of energy from fat, breast cancer risk was elevated two-fold. With respect to prostate cancer, the few existing studies show a relative consistency in supporting an association between consumption of fat containing animal products and cancer incidence. However the majority of the studies were not adjusted for total energy intake. Studies adjusting for energy intake did not find an increased risk (Kolonel, 2001).
